These tools pertain to determining the choice, needs, interests/preferences, and goals of the student in relation to the transition domains (Employment, Work, Adult Living, College/Training, and Community Connections).

Hard copy of various transition assessments are available in the chairperson's offices in the Middle and High Schools.
